| 抄録別表記: | We present in this article two cases of patients who had kindly diseases with psychosmatic complications. The two patients were followed under the Chronic Disease Clinic of the Public Health Nursing Department of St.Luke's International Hospital. In the conclusion to the paper, we discuss the method used in the clinic's management. Case 1.:the patient was brought to the hospital in a semicomatose condition with hepato-renal syndoroms. After admission he received an acute hemodyalisis. Case 2.:the patient was diagnosed as having chronic renial failure, when he first came to see the doctor. The profiles of both cases are similar;they were both 30 year old males who had been in good health previously. Both had serious characters. When the two patients were referred to our clinic after discharged from the hospital, we did not attempt to detemine their psychosocial status. We intended to evaluate their physical conditions and to encourage self-preservation in the patients. In the course of process, they developed psychosomatic symptoms;insomnia, palpitations and others. Thanks to brief psychotherapy(undertaken in case 1 by a ward resident doctor, in case 2 by a medical doctor as lesion psychiatry) these symptoms disappeared. It is possible that the patients were told that their diseases could not be cured completely. Since then they must have gradually increased their anxiety and fear, although they seemed to respond to the required care(medication diet and rest schedule) positively. Throughout these experiences, we recognized the importance of assessing the patients' conditions from a holistic of view and tried to involve family members in the patients' team medical care. Furthermore, the importance of team medical care which involves several doctors(both ward doctors and doctors at the clinic), a lesion psychiatrist, ward nurses, public health nurses, dieticians and clerks, can not be overemphasized. |
