| 抄録: | 本研究の目的は、文献研究と面接法による質的研究の結果から、がん患者が、どのような苦痛や苦悩を訴えているのかを吟味し、その現象を整理することである。 日本語文献については、がんの事例を多くとりあげた雑誌、書籍からがん患者の苦痛の訴えを抽出し、カテゴリー／サブカテゴリーを整理した。 質的研究は、東京都内のがん専門病院において、真実告知をうけたがん患者45名を対象として、患者の苦痛について、半構成的インタビューを行なった。文献から抽出された訴えと、インタビューから得られた訴えを比較したところ、それぞれ、3つの同様のカテゴリーが抽出された。その3つとは、病と向き合う過程で生じる苦痛／個人の統合性が脅かされることに関する苦痛／実存性が脅かされることに関する苦痛であった。 インタビューから得られた訴えの頻度を算出した結果、病と向き合う過程で生じる苦痛の訴えの中では治療に関連しておこる苦痛が32％でもっとも多かった。 個人の統合性が脅かされることに関する訴えの中では喪失に関する悲嘆の訴えがもっとも多く45％であった。 実存性が脅かされることに関する訴えの中では、死に対する思いや準備に関するものがもっとも多く77％を占めていた。 3つのカテゴリー間の関係性について、患者を1人の生活を営む全人的存在として理解するとき、これらのどのカテゴリーに位置する訴えが問題となっても、患者は苦痛を経験する。これら3カテゴリーが解決されない苦痛として悪循環する時、患者は苦悩を経験するのではないかと考える。 |
| 抄録別表記: | The purpose of this study was to understand the experience of cancer patient's suffering and pain through a literature review and the interview with cancer patients. The literature used for this study were mainly Japanese articles and books published in recent years, and they were mostly based on clinical case studies. Through the content analysis and classification of categories three major themes were identified:they were 1) pain and distress due to facing the disease, 2) pain and distress due to the threat to individual integrity, and 3) pain and distress due to the threat to existence. The interview were done with 45 inpatients who were informed of their disease and the majority of whom were under either the chemotherapy or X-ray therapy or both of them. The interview data were coded according to the patient's complaints of pain and distress and 396 complaints were gatherd. Those codes were classified into the same main 3 themes: 275 complaints (69.4%) were 1) pain and distress due to facing the disease, 60 complaints (15.2%) were 2) pain and distress due to the threat to individual integrity and 61 complaints (15.4%) were 3) pain and distress due to the threat existence. The experience of suffering was appeared when any of pain and distress of the above mentioned was observed, unresolved for a certain period and was accumulated. |
