| 抄録: | 本研究の目的は、老人保健施設入所者の身体的、精神的状態の変化と、変化に対する要因を明らかにし、今後の老人保健施設のサービスについて考えることである。対象は東京都にある、2か所の老人保険施設入所者29人であった。調査は、入所時と2か月後の2時点で、身体的状態と、精神的状態に関する測定尺度を用いた質問と、半構成的な質問を行い、以下の結果が得られた。 1. Barthel index によるADLの改善は統計的に認められなかった。しかし、半構成的な質問においては、実感的なレベルのADLの低下を訴えたものは7%弱にとどまったのに対し、以前より歩けるようになったなどの実感レベルのADLの改善がみられた者が約3割強と、大きな差が見られた。 2. 主観的健康度は、2か月後に統計的に有意に改善し、半構成的な質問においても、半数以上のものが、体調の維持増進について、肯定的に表現していたのに対し、否定的な表現はまったく見られなかった。 3. 精神的状態の指標として用いた、生活満足度と、自己式抑うつ度 (Self-rating Depression Scale;以下SDS得点と省す) には、入所時と2か月後の間に、統計的に有意な差はなかった。また、半構成的な質問においても、精神的側面を肯定的に表現していたものと、否定的に表現していたものとの訴え率は、約7割とほぼ同じであった。 4. 入所時から2か月後の、SDS得点の変化と有意な関連が認められたのは、主観的健康度の変化(r=-0.38, p<0.05)、および生活満足度の変化(r=-0.70,p<0.01)であり、主観的健康度が上がった者、または、生活満足度が上がった者ほどSDS得点は下がった。 以上のことから、入所者の精神面の支援が今後の老人保健施設のサービス向上の課題であると考えられた。 |
| 抄録別表記: | The purpose of this study was to identify physical and mental changes in residents in the Intermediate Health Care Facilities for elderly people and related factors,and to discuss the service provided in the facilities. A survey was cnducted in two facilities in Tokyo and data were collected from 29 residents just after admission and at 2 months post admission,using questionnaires with measurement scale and a semi-structed questionnaaire concerning their physical and mental conditions.The result was as follows: 1.No significant improvement of Activities of Daily Living(ADL) based on the Barthel Index was found statistically.However,30% of the subjects reported in response to the semi-structured questionnaire that they remarkably felt their improvement of ADL such as the stability of gait compare to less than 7% of those who felt deterioration of ADL. 2.Staying in the facility for two months resulted in statistically significant improvement in their subjective health.It was also found that 60% of the subjects gave positive expression and 6.9% gave negative expression to their physical conditions in response to semi-structured questionnaire. 3.No significant difference was found between just after admission and at two months post admission on the degree of satisfaction with their lives and Self-rating Depression Scale(SDS) which were used as an index of mental conditions.In addition,the rate of subjects giving positive expression to their mental conditions in response to the semi-structed questionnaire was 70%,which appeared to be nearly the same as those giving negative expression. 4.Change in SDS score related negatively with change in level of subjective health(r=-0.38,p<0.05) and satisfaction with their lives(r=-0.70,p<0.01). Thus,improving mental support must be the most important issue in the Intermediate Health Care Facilities for elderly people. |
