| 抄録: | 本研究の目的は、育児相談での母親と保健婦の相互作用を明らかにすることである。保健所の育児相談に来所した母親20名と相談を担当した保健婦13名、合計20事例を対象に帰納的な因子探索的研究を行った。相談場面の参加観察、相談終了後の母親と保健婦への半構成的インタビューを実施し逐語記録を作成し、分析した。その結果、育児相談での母親と保健婦の相互作用は【母親の求めることを明確化する】【母親に必要なことを明確化する】【ニーズを充足する】局面で構成されていることが明らかになった。【母親の求めることを明確化する】では、保健婦が《引き出す》、母親が《伝える》、保健婦が《捉える》という働きかけが行われていた。【母親に必要なことを明確化する】では、保健婦が《探索する》、母親が《協力する》、保健婦が《判断する》ことを行っていた。【ニーズを充足する】では、保健婦が《提供する》、母親が《得る》という働きかけが行われていた。また、相互作用の局面にはつながりがあり、【母親の求めることを明確化する】で保健婦が母親の求めることを《捉える》ことをし、それをもとに【母親に必要なことを明確化する】の《探索する》に移行していた。【母親に必要なことを明確化する】から【ニーズを充足する】へは、保健婦が母親に必要なことを《判断する》ことをし、その判断をもとに援助を《提供する》に移行していた。育児相談での母親と保健婦の相互作用は、母親の求めることと母親に必要なことを中核として展開されており、保健婦は母親が求めることだけでなく、相互作用を行う中で母親が真に必要としていることを判断しニーズを充足させる働きかけを行っていた。母親は保健婦との関わりの中で、自分の求めることだけでなく必要なことを見出され充足されていた。このような関わりあいをもつことが支援的な育児相談につながると考えられた。 |
| 抄録別表記: | The objective of this study was to investigate the interactions in child care counseling between mothers and public health nurse. An inductive qualitative study was conducted on 20 mothers and 13 public health nurse in 3 public health centers. Date were collected by participant observation without intervention in counseling sessions and semistructured interviews after counseling sessions, which were recorded on tape with the consent of the subjects. As a result of analysis, the interactions between mothers and public health nurses were classified into 3 proscesses: "clarify mother's wants," "clarify mother's needs" and "sarisfy mother's needs." "Clarify mother's wants" included three categories of actions: public health nurse draw, mother inform and public health nurse catch. "Calrify mother's needs" had three categories:public health nurse explore, mother cooperate and public health nurse judge. "Satisfy mother's needs" had two categories:public health nurse give mother get. "Calrify mother's wants" were connected with "clarify mother's needs" by public health nurse catch, and "clarify mother's need" were connected with "satisfy mother's needs" by public health nurse judge. Public health nurses acted to mothers not only according to mother's wants but also to her needs. Most of the mothers positively evaluated the counseling; therefore public health nurses' actions were considered as helpful to mothers. This study revealed that mother's wants and needs were at the core of interactions between mothers and public health nurses in child care counseling. It is cinsidered inportant to clarify and satisfy mother's wants and needs in developing interactions between mothers and public health nurses. |
