| 抄録: | 在宅で悪性症候群を発症した精神障害者への援助を通して、1)看護職(保健婦)による在宅精神障害者の症状･副作用のモニタリング機能と、2)保健と福祉の連携･共働のあり方について考察し、3)保健と福祉の連携･共働時の看護職(保健婦)の機能の特質を明らかにするために事例検討を行った。 事例検討の情報は、訪問指導記録、また、保健婦の援助中の参加観察によって情報収集した。 事例の概要:事例は60歳の単身男性。3ヶ月あまりの入院治療後、退院。診断は器質性精神障害であった。今回の入院以前は、時々日雇いの仕事に従事していた。退院後1週間で悪性症候群が発症し、ヘルパーからの緊急要請で訪問し、受診に至った事例である。 検討の結果、以下のことが明らかになった。 1)在宅精神障害者の症状･副作用のモニタリング機能においては、(1)服薬による身体状態や患者の訴えをモニタリングし、(2)緊急性を見極めるとともに、(3)必要時適切な医療機関につなげる援助が重要である。 2)保健と福祉との効果的な連携･共働のためには、早い段階で担当者双方の顔合わせをすることにより互いに連絡をとりやすい基盤を作ること、福祉職に症状･副作用のモニタリングの知識を体得してもらうこと、さらに適切な共働のタイミングを双方ではかることが重要である。保健と福祉の連携･共働時の看護職(保健婦)の機能の特質は、医学･病態生理学的根拠に基づいた援助技術と知識の提供があげられる。 今後は、患者の情報を統合し、症状のモニタリング機能の充実と情報伝達システムの確立を図っていく必要がある。 |
| 抄録別表記: | The following is a case study through nursing of psychiatric patient cared at home who has fallen in neuroleptic maligant syndrome.It was conducted to exermine 1)monitoring function by the public health nurse regarding the syndrome and by-effect on the psychiatric cared at home and to clarify 2)the specific function of the public health nurse under cooperation between public health and welfare functons. The data was obtained from the record of visit instruction and actual observation while the public health nurse was at work. Outline of the study:The patient is a 60-year-old male living alone.After being cared at hospital over three months,he returned home.The diagnosis was physical nature mental disease.Before being hospitalized he was sometimes engaged in daily-employment work.After one week of returning home,neuroleptic malignant syndrome occurred with him.The helper made an emergency request and he received a checkup. The following points were made clear: 1)Regarding the monitoring function of syndrome and by-effect on the psychiatric patient cared at home,it would be necessary;(1)to monitor physical condition and appearls from the patient after medicine is given,(2)to judge the degree of emergency and (3)to offer assistance to arrange for appropriate medical institutions when it is necessary. 2)Regarding effective association and cooperation between public health and welfare functions,there are three important factors;the persons in charge should be acquained at an early stage to facilitate communication,the welfare function should accurie knowledge to monitor syndrome and by-effect and both functions should try to work at an appropriate timing for cooperation.The specific function rendered by the public health nurse is to provide assistance and knowledge based on medical and pathophysiological background. It would be necessary to establish an appropriate monitoring system of patient's syndrome and effective information flow system by integrating patient information. |
